Seeks Statewide SSVF Healthcare Navigator

Community Action Partnership of North Dakota has an opening for a full-time statewide Supportive Services for Veteran Families (SSVF) Healthcare Navigator. The SSVF Healthcare Navigator provides services to connect Veterans to VA healthcare benefits or community healthcare services. The SSVF Healthcare Navigator works closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team. Direct responsibilities include, but may not be limited to, providing case management and care coordination, health education, interdisciplinary collaboration/coordination/consultation, and administrative duties.
The position requires an LSW with an MSW preferred, or other equivalent education and experience. Minimum of two to five years of experience in human service field along with knowledge of healthcare systems, coordination, and case management. Excellent computer and communication skills are a must.
This position requires extensive travel.
