CAP Utah - 2014 Awards Nomination Packet

The Community Action Partnership of Utah is accepting nominations of exceptional individuals or organizations who have demonstrated dedication and leadership in preventing and eradicating poverty in Utah over the last year.  Award recipients will be honored at the 7th Annual Utah Community Action Awards Luncheon being held on Wednesday, May 7th.

Local award winners in these categories will be submitted to Community Action Partnership for consideration on these nationwide awards.

CAP Utah Award Categories

Community Partner of the Year (Organization/Individual)
This award recognizes a community partner that has been essential to your agency’s success this past year

Excellence through Community Action – Urban (a volunteer, staff or board member)
This award recognizes someone from your staff, board or volunteer base who has demonstrated excellence in their work and has embodied the spirit of Community Action.

Excellence through Community Action – Rural (a volunteer, staff or board member)
This award recognizes someone from your staff, board or volunteer base who has demonstrated excellence in their work and has embodied the spirit of Community Action.

Utah Earn It. Keep It. Save It. Volunteer of the Year
The purpose of this award is to honor an outstanding volunteer in the EIKISI coalition.

Public Official of the Year (local, state or national)
This award recognizes a public official who has played an important role in supporting your organization’s work over the past year.

Community Action Emerging Leader Award
Nominate an outstanding young person in Utah’s Community Action Network who deserves recognition for their leadership qualities and dedication to the mission of Community Action.  This person can be an employee or volunteer of a Community Action Agency.

National Awards

2014 Sargent Shriver Achievement Award

The individual nominated must have:
  • Changed his or her life and achieved economic self-sufficiency with the assistance of a Community Action Agency; and
  • Contributed to improving his or her community through advocacy, volunteerism, public service, or some other vehicle

2014 Jayne Thomas Grassroots Volunteer Recognition Award

The candidate must:
  • Be a volunteer for a Community Action Agency
  • Have made a significant and outstanding contribution toward accomplishing the promise of Community Action
  • Be devoted to changing people’s lives
